Registration

To register for Buckeye Baseball 18+, please click on the Baseball buttons below to download the League Affiliation Form, the League Roster Form, and the instructions on how to register players so they will be eligible to compete in the State Championship playoffs.
The League Affiliation form must be completed and set to Buckeye Baseball 18+ along with the membership check covering $25 per team, minimum $150.
The League Roster form should be completed at the same time and included with the League Affiliation form.
The forms below are in PDF format.  If you do not have Adobe Acrobat, the base version is free and may be downloaded by clicking on the YELLOW button below the forms buttons.  Download the forms, print them out, complete them and return them to Buckeye Baseball 18+ with your remittance.
